Online registration is straight forward. Choose a link on the sidebar to begin.

1) Follow the instruction on the form to proceed with registration.

2) Click the SUBMIT button once the form is filled completely. Registration is done.

3) To Edit the form, click on EDIT YOUR RESPONSE link.

4) Your response will be editable.

5) Copy the URL and save it in a word processing software such as NOTEPAD for future use if needed.

NOTE: You must copy the URL before closing the page as the URL cannot be recall after this.
